Thyroid eye disease: diagnosis and management

Hyperthyroidism is a common condition mainly affecting the female population. The majority of individuals with hyperthyroidism have Graves’ disease (GD), an autoimmune condition characterised by the presence of thyrotropin receptor stimulating antibodies. Extrathyroidal manifestations of GD may occur in up to 50% of patients, in the form of thyroid eye disease (TED) and less commonly pretibial myxoedema. TED, an inflammatory condition of the orbit, is usually mild but can be severe and sight-threatening. The condition is often misdiagnosed resulting in delays in initiation of appropriate therapy with potentially adverse clinical consequences. This presentation will address the clinical aspects of TED, including presenting symptoms and key signs found on examination. Furthermore, laboratory and radiological diagnostic tests will be discussed together with the referral criteria to a specialised service. Medical and surgical management strategies, including treatment of difficult cases, will also be reviewed.
