Introduction: Hirata Syndrome (SH) is a rare cause of autoimmune hypoglycemia, characterized by elevated circulating insulin levels without concomitant increase in C-peptide, anti-insulin antibodies positivity and pancreas alterations absence in patients never exposed to exogenous insulin.
