Dysgerminoma and gonadoblastoma in children with complete androgen insensibility

Children with androgen insensibility and female sexual external organs usually are classified as girls. In the puberty observed is the development of female secondary sexual features without menstruation. The treatment of patients with androgen insensibility, because of the risk of neoplastic transfomation must be preceded by castration. Case report: a 14 year old girl was admitted in 2003 to the Clinic with the following anamnesis: birth from the secondary gravidity, normal delivery. In 2002 admitted to the Pediatric Clinic of Surgery because of a dramatic increase of the abdomen circumference. On the 23.06.2002 a surgical intervention was performed during this a tumor of 4000 g, with was grooving up from the right gonad. The histopathological examination ascertained a dysgerminoma partim necroticans.

The examined kariotype proved an XY [24]/ 87 XXYY, dic(1;?10;)(p13;?p15), dic(1;7;6)(pterp13;p15q13pter), dic(12;?20)(p24.3;?),mar[1] In 12.10.2002 admitted to the Surgery Clinic because of a neoplastic process in the left gonad, which was removed. After the first surgery admitted to the Pediatric Clinic if Hematology and Oncology where she was treated until 2003. The histopathological examination ascertained gonadoblastoma. The laboratory results at the time of admission: LH-48.0 mIU/ml, FSH-107.0 mIU/ml, TTE-0.24 ng/ml, Estradiole <20.0 pg/ml, DHS-60.6 ug/ dl. The results now: LH- 155. 0 mIU/ml, FSH- 62.5 mIU/ml, TTE-0.34 ng/ml, Estradiole < 20.0 pg/ml, DHS-122.0 ug/ dl. Bone age 12.5/12 years, TSH-2.24 uIU/ml, fT3-3.6 pg/ ml, fT4-1.4 ng/dl, ß HCG-4.1 mIU/ml (n-0-2.7), densytometry – 24% decrease of the bone density in comparison with the calendar age (2 score). Body mass 51.7 kg (50 cent) height 151 cm (10 cent). Gynecological examination: labia maius end minus pudenda developed normal. Clitoris of normal size. The vaginal admittance normal, the vagina blindly ended. The status after bilateral castrations.
