Background: Primary hyperparathyroidism (PHPT) is one of the most common endocrine diseases. Parathyroid carcinoma, a rare entity, comprises a small percentage of patients with hyperparathyroidism and represents 0.005% of all cancers and <1% of cases of PHPT.

Material and methods: Patients was diagnosed at the Endocrinology Department of Diskapi Yildirim Beyazit Training and Research Hospital between 2010 and 2016. Ultrasound and 99mTc-Sestamibi confirmed the suspicion of a parathyroid mass. Parathyroid gland volumes were calculated by multiplication of three diameters and the constant value 0.52. Intraoperative findings and pathology confirmed the diagnosis of parathyroid carcinoma.

Results: Of the patients eight were female and three were male. The patient’s ages were between 27–76 years. Parathyroid gland volume of two patients were lower than other cases (respectively 8.92–10.17 ml). Despite small lesion size on ultrasonography, these two patients have moderate-high PTH values. Conclusion: Pre-operatively, parathyroid carcinoma can be expected based on factors including a larger tumor size, palpable mass, severe primary hyperparathyroidism (PTH 3–15 times the normal upper limit) and hypercalcemia (>14 mg/dl) often with renal and bone involvement by the onset of presentation. Biochemical or ultrasonographic features (lesion size, very high serum calcium and parathyroid hormone levels) may predict aggressive disease, but this is not always the case.
