Composite Pheochromocytoma with Adrenocortical Carcinoma - a rare coexistence

Sarwani Aysha , Morrin Martina , Naidoo Jarushka , O'Brien Odharnaith , Skordilis Kassi , Ryan Cliona , Dugal Neal , Hill Arnold , Srinivasan Shari , Sherlock Mark , O'Reilly Michael

A 65-year-old man presenting with urinary symptoms was incidentally discovered to have an adrenal mass on imaging. Abdominal computed tomography (CT) revealed a 9.5 x 8 cm heterogenous solid right adrenal tumour with no evidence of metastatic disease. Family history included a brother who died from metastatic carcinoid and another brother who died from metastatic neuroendocrine tumour. The initial differential lay between adrenocortical carcinoma (ACC) or a pheochromocytoma. T...

Composite Pheochromocytoma with Adrenocortical Carcinoma - a rare coexistence

Sarwani Aysha , Morrin Martina , Naidoo Jarushka , O'Brien Odharnaith , Skordilis Kassiani , Ryan Cliona , Dugal Neal , Hill Arnold , Srinivasan Shari , Sherlock Mark , O'Reilly Michael

A 65-year-old man presenting with urinary symptoms was incidentally discovered to have an adrenal mass. Abdominal computed tomography (CT) revealed a 9.5x8cm heterogenous solid right adrenal tumor with no evidence of metastatic disease. The initial differential lay between adrenocortical carcinoma (ACC) or a phaeochromocytoma. The elevated plasma fractionated normetanephrine at 14439 pmol/l (0-1180), metanephrine 15202 pmol/l (0-510), 3-MT 415 pmol/l (0-180) led to a presumed ...

Thyroid malignancy rate in U3 thyroid nodules

Al Busaidy Merah , Smith Diarmuid , Sherlock Mark , O'Reilly Michael , Wanic Krzysztof , Hill Arnold , O'Neill Paul , Morrin Martina , Dunne Ruth , Canning John , Ryan Cliona , Agha Amar

Background: Thyroid nodules incidentally picked up on imaging studies are extremely common with a prevalence of around 50% in the general population. Ultrasound characterization using the British Thyroid Association (BTA) U classification system is commonly employed in the United Kingdom and Ireland. Most reported nodules are classified as U3 which indicates an indeterminate suspicion of malignancy. There are limited data on the malignancy rates in U3 nodules.<p class="abs...

Is repeat fine needle aspiration required in thyroid nodules with initial benign cytology?

Zia-Ul-Hussnain Hafiz Muhammad , Quinn Mark , Dolan Emma , Sherlock Mark , Thompson Chris , Smith Diarmuid , O'Neill James Paul , Leader Mary , Barrett Helen , Hill Arnold , Morrin Martina , Ryan Cliona , Agha Amar

Background: Fine needle aspiration (FNA) is the preferred method for assessing thyroid nodules but concern remains about false negative results. The primary aim of this study was to investigate the malignancy rate in nodules which were initially classified as benign (BTA classification Thy 2). The secondary aim was to look at the distribution of different cytological categories in a large cohort of patients. Methods: We retrospectively examined 719 nodule...

Metabolically unhealthy obese patients have decreased and altered circulating natural killer cells and increased adipocyte size compared to obese metabolically healthy patients

Lynch Lydia , O' Connell Jean , Kwasnik Anna , Cawood Tom , O'Farrelly Cliona , O'Shea Donal

Aim: We propose that the circulating immune system and adipocyte size plays a role in the development of obesity related co-morbidities independent of body mass index (BMI).Methods: Clinical data and blood samples were collected from patients with severe obesity attending a hospital weight-management clinic and lean healthy controls. Patients were classified into metabolically ‘healthy obese’ or ‘unhealthy obese’ groups, based upon st...

An audit of electronic consultations for provision of endocrine specialist advice

Costa Ryan D

Background: Nationally, there has been a concerted drive to utilise technology to support the provision and delivery of specialist services. Within our local area, both secondary care and 95% of GP surgeries utilise the Systm-One electronic health care record. Since October 2016, we have been providing a local Endocrine E-consultation service. A GP requiring advice, rather than referring the patient to the hospital would (after obtaining patient consent), share their record, t...

Two hour insulin/C peptide levels in oral glucose test: are they really necessary?

Anuar Azriyanti , Ryan Fiona

Introduction: A standard oral glucose tolerance test (OGTT) measures samples at 0, 30, 60, 90, 120 min. Insulin and c-peptide are taken along with blood glucose (BG). Whether samples between baseline and 120 min provide any additional value in detecting children at high risk in clinical practice is not well documented. This audit is looking at the value of doing insulin and c-peptide within 2 h post glucose ingestion in otherwise healthy children seen in Oxford Children’s...

Audit of children with thyrotoxicosis treated with antithyroid drugs by block and replacement regime: relapse rate and outcomes

Rajput Shailendra , Ryan Fiona

Introduction: Drug-based therapy is usually the initial treatment for thyrotoxicosis in children, but there is some debate about treatment duration. Our objective was to assess the effect of long-term Carbimazole therapy by block and replacement regime on thyrotoxicosis remission in children. Methods: 15 children with thyrotoxicosis seen in endocrine clinic between January 2006 and January 2010 were included in the audit. Data was collected retrospectivel...

Comparison of existing practice of FNA cytology in thyroid nodules against the standards

Hafeez Kashif , George Ryan

Introduction: FNA cytology are done for solitary nodule or a dominant nodule in MNG.FNA classified in 5 groups (THY1 to THY5). FNA labelled as THY1 and THY2 should have repeat FNA. We have studied the FNA cytology over the period of 5 years. They are done in clinical settings by physician, surgeon or radiologist. Aim: To assess the trust practice of FNA cytology in Thyroid swelling and compare them with standards. Methods: Data were collected from patholo...

Thyrotoxic crisis: a stormy period on intensive care

Eapen Dilip , D'Costa Ryan

Thyroid storm or thyrotoxic crisis is a manifestation of an extreme state of thyrotoxicosis. It is both rare and potentially fatal. Its presentation can be clouded by the precipitating illness and the involvement of one or more organ systems underlining the importance of clinical suspicion, early recognition and prompt intervention. We present a case of a 37-year-old man presenting to hospital with a community acquired chest infection that had not settled...
