A managed clinical network for thyroid disease

Cuthbertson D , Jung R , Leese G

The high prevalence of thyroid disease necessitates efficient and effective monitoring. We expanded a hospital-based register to a managed clinical network for almost 10,000 patients with thyroid disease. Evaluation of our network compared to register data from 1991 demonstrates changing trends in treatment of hyperthyroidism. Use of radio-iodine significantly increased (58 percent v 44 percent; p less than 0.001) with a reduction in thyroid surgery (34 percent v 47 percent; p...

Immunology of implantation and pregnancy

Sargent I , Sacks G , Redman C

Recent advances in reproductive immunology have caused a fundamental shift in our appreciation of the nature of human pregnancy. For nearly 50 years, the subject has been dominated by Medawar's 'fetal allograft' hypothesis, in which an analogy is assumed between the fetus and a tissue transplant. In this framework, all maternal immune mechanisms are considered potentially harmful for the fetus and, therefore, must be overcome, bypassed or suppressed. Much progress has been mad...

DOES GROWTH HORMONE REDUCE SENSORINEURAL DEAFNESS IN ADULTS WITH TURNER SYNDROME?

Ostberg J , Beckman A , Cadge B , Conway G

IntroductionWomen with Turner Syndrome have an increased prevalence of hearing loss. Conductive hearing loss (CHL) is thought to be caused by craniofacial abnormalities or defective mucociliary transport resulting in impaired middle ear ventilation, increased otitis media and tympanic perforations. Sensorineural hearing loss (SNHL) progresses with age, possibly due to premature aging. An adult population was assessed for the impact of childhood treatment...

UNUSUAL PRESENTATION OF DIABETIC FOOT ULCER

Mousa A , Knight G , Muzulu S , Rochester J

UNUSUAL PRESENTATION OF DIABETES FOOT ULCERAM Mousa, G Knight, S Muzulu, J R Rochester. Rotherham General Hospital, Moorgate Road, Rotherham S60 2UD.IntroductionPhalangeal metastases commonly display inflammatory symptoms that mimic an acute infection. We describe a rare case of bronchogenic carcinoma with metastases to the left 3rd and 4th toes, which presented as presumed diabetic foot ulceration.<p cla...

Growth hormone deficiency in salt-losing congenital adrenal hyperplasia

Traggiai C , Conway G , Stanhope R

Introduction: Congenital adrenal hyperplasia (CAH) is characterised by impaired gluco and mineralocorticoid and excessive adrenal androgen secretion from the adrenal cortex. More than 90% of cases of CAH are caused by defects in the steroid 21-hydroxylase (CYP21) gene. We describe the association between salt-losing 21-hydroxylase deficiency and growth hormone deficiency.Patients and Methods: We reviewed 4 patients with salt-losing CAH (CYP21), who pres...

An androgen-secreting tumour presenting with resistant hypertension and an 'incidentaloma'

Kong M , Moleele G , King P

A 77 year old lady was referred by her GP after she was found to have a 6x5x5 cm diameter solid mass in the right adrenal gland on ultrasound scan. The mass showed significant internal vascularity on colour doppler and the radiologist felt it could be aphaeochromocytoma. She had a long history of hypertension which her GP had been finding difficult to control over the past 2 years.Type 2 diabetes was diagnosed 2 years ago and she...

Lectin binding of serum hCG in pregnancy and type 1 diabetes

Nayar R , Dillon S , McNab G , Chapman A

Advanced glycation end products modify protein structure and function in type I diabetes. Glycoproteins may be potentially modified through other mechanisms, particularly changes to insulin-dependent oligosaccharide components. We have assessed the use of immobilised lectins, characterising the oligosaccharides of a serum glycoprotein, human chorionic gonadotrophin (hCG). We wish to test the hypothesis that glycoprotein glycosylation may be modified in type I diabetes.<p ...
