Four cases of thyroid carcinoma presenting in childhood: 15 years experience in a National Tertiary Referral Centre

Ervine Evelyn , Wallace Ian , Taghavi Kiarash , Morreau Philip

Thyroid cancer has an annual incidence of 0.2–5 per million children representing 3% of all childhood tumours. We describe the presentation, histopathology and treatment of four patients in a regional paediatric surgical and endocrine unit over the past 15 years.Three of four cases are female with 1 male. Age of diagnosis ranged from 6 to 15 years. All had an elevated thyroglobulin at presentation with normal thyroid function tests. Three underwent ...

TSH-secreting pituitary adenoma: potential benefits of pre-operative octreotide

Wallace Ian , Healy Estelle , Cooke Steve , Harper Roy , Hunter Steven

TSH-secreting pituitary adenomas are rare and the optimal investigation and management is uncertain.A 42-year-old lady presented with a three-month history of three stone weight loss, palpitations, heat intolerance and tremor. Her sister was being treated for Graves’ disease. Visual fields were intact.Thyroid function tests showed free T4 concentration 29.5 pmol/l (9.0–19.0) and TSH concentration 3.672 mU/l (0....

Challenges in management of primary hypoparathyroidism associated with autoimmune polyglandular syndrome type 1 (APS I)

Wallace Ian , McConnell Vivienne , Bell Patrick , Lindsay John

APS I is a rare condition with autosomal recessive inheritance. We report a series of three sisters (Table 1), affected by APS I, who all had a mutation of the autoimmune regulator (AIRE) gene. An index case (A) died at 26 years old due to vascular complications. The management of primary hypoparathyroidism in case A, who also had type I diabetes was also complicated by nephrocalcinosis, renal calculi, chronic renal failure and vascular calcification. In ...

Confusion in a patient with Carcinoid syndrome

D'Arcy Robert , McCance David , Johnston Brian , Eatock Martin , Wallace Ian , Graham Una

We present a 66 year old man with a metastatic small bowel neuroendocrine tumour managed with resection of primary in 2011 and subsequent somatostatin analogue therapy. In 2018 disease progressed requiring right ureteric stenting due to extrinsic nodal compression. Three weeks later he presented with sweats, pyrexia and elevated inflammatory markers. Multiple blood and urinary cultures were clear. CT imaging, MRI spine and echocardiogram did not identify a source of sepsis. Ur...

Vitamin D and insulin resistance: no association in healthy overweight people at high risk of cardiovascular disease

Wallace Ian , McEvoy Claire , Hamill Lesley , Ennis Cieran , Bell Patrick , Hunter Steven , Woodside Jayne , Young Ian , McKinley Michelle

Observational studies suggest reduced vitamin D levels are associated with an increased incidence of type 2 diabetes mellitus. We examined the relationship with insulin resistance (assessed using a two-step euglycaemic hyperinsulinaemic clamp technique) in 92 overweight, non-diabetic individuals with no history of cardiovascular disease - mean age 56 years (range 40–77 years), 64% males, 36% females, BMI 30.9 kg/m2 (range 26.4–36.9 kg/m2), fasti...

Association between low concentration of serum sex hormone binding globulin and insulin resistance is independent of adiposity, but may be attributable to fasting insulin concentration

Wallace Ian , McEvoy Claire , Hamill Lesley , Woodside Jayne , Ennis Cieran , Bell Patrick , Young Ian , McKinley Michelle , Hunter Steven

Introduction: Recently low circulating levels of sex hormone binding globulin (SHBG) have been shown to be a strong predictor of the risk of developing type 2 diabetes. Genetic studies suggest that this may be a primary causal abnormality and the mechanism may relate to effects on insulin resistance. Methods: Insulin resistance was assessed using a two-step euglycaemic hyperinsulinaemic clamp (EHC) in 87 (59 male and 28 post-menopausal female) overweight ...

Selenium (Se) supplementation is recommended by the European Group (EUGOGO) for mildly active TED (200 mg/day for six-months) based on a seminal RCT which reported slowing of disease progression vs placebo. Treatment in Se deficient geographical areas may be particularly important. We recently reported Se deficiency in a cohort of 240 pregnant women in NI. The TEAMeD-NI group was set up in 2019 as part of the UK-wide quality improvement program to take a proactive approach to ...
