Influence of pre-treatment factors on outcomes following the use of high fixed dose radioiodine treatment for hyperthyroidism

Barton D , Buch H , Baskar V , Kumar H , West T

Radioiodine therapy (RAI) is widely used as definitive treatment for hyperthyroidism. Of the several regimes in use, we have used a standard 555 MBq dose of RAI to treat all patients with hyperthyroidism. Treatment is considered to have failed if patients remain hyperthyroid at 12 months and a repeat 555 MBq dose is administered. We have studied 584 consecutive patients for failure rate of RAI when it is used according to this regime (mean follow-up 3 years (range 1-10)). We h...

1,25-HYDROXYVITAMIN D3 HAS A DIRECT EFFECT ON STEROID PRODUCTION BY HUMAN GRANULOSA CELLS

Brain H , Bano G , Mason H , Nussey S

1,25-hydroxyvitamin D3 receptors (VDR) are present on rat granulosa cells and on human ovarian cancer cells, however little is known about 1,25-hydroxyvitamin D3 (1,25-(OH)2D3) function in the ovary. Vitamin D deficient rats have reduced fertility, and VDR null mutant mice demonstrate impaired folliculogenesis. Vitamin D metabolites have been detected in follicular fluid. The aim of this study was to investigate the effect of a range of conce...

Reduced fatigue and improved sexual function in hypogonadal men treated with TESTIM in routine daily practice

Behre H. , Tolle A. , Gedamke M.

Introduction: Hypogonadal men frequently suffer from fatigue and reduced sexual function. International guidelines recommend testosterone replacement therapy (TRT) for symptomatic hypogonadal men. Our aim was to document testosterone changes and the impact of TRT with TESTIM on quality of life in routine daily practice. Methods: In this non-interventional-study (NIS) data from 117 hypogonadal patients from 43 office-based-study-sites were evaluated. Data ...

Quantitative comparison of deiodinase I and II mRNA expression in ovine tissues

Dehghani H. , Foroughi abari M.

Iodothyronine deiodinases are important enzymes for metabolism of thyroid hormones. These enzymes which are expressed in a variety of tissues are able to catalyze removal of iodine from thyroid hormones. Iodothyronine deiodinases I and II (DIO1 and DIO2) remove iodine from T4 to convert it to a more biologically active T3. The relative contribution of different tissue deiodinases to the establishment of a euthyroid state (proper production and concentration of T3) is not known...

Imaging of pheochromocytoma and paragangliomas

Timmers H J L M

The standard technique for the functional imaging of pheochromocytoma and paraganglioma (PPGL) is [123I]-metaiodobenzylguanidine single photon emission computed tomography (MIBG SPECT). However, metastases are more accurately detected by 2-[18F]-Fluoro-2-deoxy-D-glucose positron emission tomography (FDG PET). In a large prospective study of 216 patients evaluated at the NIH for benign or malignant PPGL, we investigated the sensitivity and specificity of FDG PET ...

Audit on follow up of thyroid function tests done during osteoporosis screening

Santhakumar A , Sugathan H , Woods D

Background: National osteoporosis guideline group recommend routine measurement of thyroid function tests. However it is well recognised that abnormal thyroid function tests done during acute hospital stays is not always suggestive of thyroid dysfunction and that they need to be repeated to determine the need for further management.Aim: To identify and assess the follow up of thyroid function tests (TFTs) done between June 2009 and Feb 2010 in the orthop...

Lithium associated hyperparathyroidism: when to investigate?

Ballav C , Brain H , McLaren A

A 64-year-old woman who had been on Lithium 600 mg/day for 6 years for affective neurosis, presented in 2005 with acute confusion associated with a serum calcium concentration of 3.02 mmol/ which improved with rehydration. She was followed-up in outpatients and diagnosed with ‘Lithium-associated hypercalcaemia’ likely to be secondary to parathyroid hyperplasia. Results showed a corrected calcium of 2.74 mmol/l (2.1–2.55), phosphate 1.00 mmol/l (0.74–1.52), ...

Adrenoleukodystrophy presenting with hypoadrenalism: can it occur at any age?

Creedon H , Yogendra A , Jennings AM

X-linked adrenoleukodystrophy (ALD) is a rare condition affecting the nervous system and adrenal cortex that typically presents in childhood. It results from mutations in the ABCD1 gene. We describe two cases of ALD presenting with hypoadrenalism in adulthood.A 20 year old man presented with persistent vomiting. He was pigmented, blood pressure was 100/60 mmHg, and neurological examination was normal. Investigations showed serum sodium 125 mmol/L, urea 1...

Audit on management of chronic adrenal insufficiency

Rao Rk , Brahma A , Gray H

Background: Glucocorticoid replacement is life saving in adrenal insufficiency. There are established guidelines for management of adrenal insufficiency. This audit was under taken to evaluate whether we are implementing these guidelines in our day to day management of patients with adrenal insufficiency.Method: A retrospective study was performed on 18 patients. Data was collected from clinical records on the proforma and were evaluated against the reco...
