11 Beta-hydroxysteroid dehydrogenase 2 activity in septic shock before and during treatment with hydrocortisone

Lloyd LA , Wayte A , Griffiths K , Thorpe CM , Taylor NF , Wilton A

Hydrocortisone treatment in septic shock promotes shock reversal and reduces mortality. We hypothesised the doses that used (200–300 mg/2 h) would result in plasma cortisol levels sufficient to saturate the 11 beta-hydroxysteroid dehydrogenase 2 enzyme (11 beta-HSD 2) with cortisol inactivation overload. Consequently cortisol would access the mineralocorticoid receptor producing mineralocorticoid effects. Hypertensive metabolic alkalotic effects would tend to counteract t...

Apoprotein B and the likelihood of myocardial infarction among persons with reduced kidney function in the U.S. population: the third national health and nutrition examination survey (NHANES III)

O’Shea LA , Nguyen HT , Murthy BVR , Stack AG

Apolipoprotein (Apo)-B is a strong predictor of coronary risk in the general population. Whether it contributes to coronary disease in patients with reduced kidney function is unclear. We examined the prevalence and relationship of Apo-B with physician-diagnosed myocardial infarction among 7,404 adults age >20 years with reduced glomerular filtration rate (GFR) in the U.S. population. GFR was estimated Modification of Diet in Renal Disease Study (MDRD) equation and calibra...

The role of MRAP in the functional expression of the melanocortin 2 receptor

Cooray SN , Chapple JP , Metherell LA , Cheetham ME , Clark AJL

Familial Glucocorticoid Deficiency type 2 (patients with normal MC2R) is associated with mutations in the MRAP (Melanocortin Receptor Accessory Protein) gene. In order to investigate the function of this novel single transmembrane domain protein, CHO and SKN-SH cells were transfected with MRAP-FLAG and/or MC2R-GFP constructs and imaged using confocal microscopy. Although the MC2R-GFP failed to be expressed at the cell surface when transfected alone, it was found to be expresse...

Prolonged expression of the ACTH receptor, MC2-R, during 3T3-L1 adipocyte differentiation is dependent upon switching promoter usage to an adipocyte-specific, C/EBP-driven, downstream promoter

Noon LA , Clark AJL , O’Shaughnessy PJ , King PJ

The peptide hormone ACTH stimulates lipolysis and suppresses leptin production in adipocytes via the G-protein coupled seven transmembrane receptor, MC2-R. We have shown previously that PPARγ2 is the primary factor responsible for transactivation of the already identified murine MC2-R promoter in the differentiating 3T3-L1 adipocyte cell line. In this study we show that despite the activity of this promoter being transient during differentiation, MC2-R mRNA remains elevat...

Prevalence of classical forms of 21-hydroxylase deficiency in Western Siberia according to the results of neonatal screening of congenital adrenal hyperplasia

Suplotova LA , Hramova EB , Makarova OB , Yuzakova NU , Barcova TV

The purpose: to study prevalence of classical forms of 21-hydroxylasae deficiency according to the results of neonatal screening on a congenital adrenal hyperplasia (CAH) in Tyumen, Russia. The analysis of neonatal screening on CAH results in Tyumen for the period of 2003–2004. For the period under research 20011 newborn have been tested on the 17-OHP level. Measurement of 17-hydroxyprogesterone in blood samples was done in the central laboratory by ...

Thyroid function in patients with atrial fibrillation

Bystrova TV , Troshina EA , Abdulchabirova FM , Panchenkova LA

Background: Thyrotoxicosis is a common disorder with a prevalence of 3% in females and 0.3% in males in iodine replete areas. The prevalence may be higher in areas of iodine deficiency. It is known to induce many cardiovascular effects. The most important complication of thyrotoxicosis is atrial fibrillation. We aimed to assess thyroid function in patients with atrial fibrillation and determine the frequency of thyrotoxicosis in this group. Methods: We st...

Correction of hypomagnesaemia improves the symptoms of hypocalcaemia after thyroidectomy

Adelhamid MFA , Barsoum MS , Shaheen SA , Mansour LA

Introduction: The objective of this study is to detect whether the correction of hypomagnesaemia would help to relief the symptoms of hypocalcaemia that occurs after thyroidectomy. Methods: This prospective study comprised 35 patients who had thyroidectomy between September 2001 and June 2002. Thirteen patients had total thyroidectomy (TT), thirteen had total lobectomy on one side and subtotal lobectomy on the other side (TLSL) and nine patients had hemi...

Futher results of treatment and outcomes in patients with subacute thyroiditis observed in the period from 1993 to 2005

Petrova EV , Platonova NM , Kabelnitskaya LA , Troshina EA

Aim: To estimate further results of treatment and outcomes in patients with subacute thyroiditis observed in the period from 1993 to 2005 years.Materials and methods: 102 patients (84 women, 18 men) with confirmed De Quervain’s subacute thyroiditis (SAT) were investigated and the findings were analysed retrospectively (clinical features, ultrasonography, laboratory tests). Ultrasound device (Hewlett Packard Image Point HX) with 7.5 – 10 MHz. Se...

Cognitive impairment and nutritional status in patients admitted to the Department of Internal Medicine

Merida De la Torre Francisco J , Bel Pena Nieves

Introduction: Altered nutritional status is a problem in itself that can complicate a patient’s disease. In our hospital, the assessment of nutritional status at hospital admission is protocolized and performed by the Clinical Laboratory automatically, through the CONUT system, for three years ago. Our hospital provides assistance to a population of 120,000 patients, from rural areas and with a high average age. Hospital admissions for cognitive impairment are increasing ...

Assessing the risk of atrial fibrillation in hypothyroid women prescribed with liothyronine: A retrospective cohort study

La Cour Jeppe , Kristensen Soren , Nygaard Birte , Selmer Christian

Background: Whether combination therapy of levothyroxine and liothyronine increases the risk of atrial fibrillation remains a topic of debate. Objectives: The objective of this study was to investigate the risk of atrial fibrillation in individuals with hypothyroidism treated with a combination of levothyroxine and liothyronine. Methods: We conducted a retrospective cohort study using individual-level linkage of Danish nationwide reg...
