Type 2 diabetes

Ehtisham Sarah

Over the last 20 years, the phenomenon of type 2 diabetes in childhood has been increasingly recognised. National Audit data shows that in the UK there remains a relatively low prevalence of childhood type 2 diabetes, whilst the incidence has increased sharply in other parts of the world. This presentation aims to cover the pathogenesis of type 2 diabetes, its presentation, investigation, diagnosis and management, with a discussion of some of the newer tr...

Paediatric diabetes survey shows type 2 diabetes prevalence 0.4%, distinct from type 1, and associated with overweight, puberty, female sex and ethnic minority status

Barrett T , Ehtisham S , Smith A , Hattersley A

Most children with diabetes are insulin deficient (type 1). We have reported type 2 diabetes in children, defined by evidence of insulin resistance, but the prevalence and characteristics in UK children is unknown. We aimed to characterise and determine the prevalence of non-type 1 diabetes in the UK. The British Society for Paediatric Endocrinology and Diabetes Clinical Trials unit undertook a cross-sectional questionnaire survey of all paediatric diabetes centres during 2000...

Assessing aortic dilatation using aortic sized index is inappropriate in children and adolescents with Turner syndrome

Wong S C , Ehtisham S , Cheung M , Zacharin M

Background: Aortic sized index (ASI) defined as aortic dimensions/body surface area (BSA), has been proposed as a method of identifying aortic dilatation in Turner syndrome. A recent paper reported centile charts of aortic dimensions across for BSA using echocardiogram in 451 children and adults with TS allowing for calculation of Z scores.1Methods: We report Z scores for aortic root adjusted for BSA from clinical echocardiog...

Visual disturbance in diabetes mellitus: don't be blind to alternatives to retinopathy

Smith Timothy , Mukerjee Anindya , Ehtisham Sarah , Tomlinson Naomi

Introduction: We present the contrasting cases of two siblings diagnosed with Wolfram’s syndrome aged 2 and 10 years old. We discuss several family factors which have made management of this rare condition even more complex, emphasising the importance of holistic medicine.Case: The older sibling was diagnosed with diabetes mellitus age 4. Parents struggled to cope with the diagnosis, however, and emotional and psychological support was offered. She ...

A family kindred with persistent Mullerian duct syndrome secondary to AMH deficiency

Farikullah J , Nappo S , Hennayake S , Patel L , Ehtisham S

Background: Persistent Müllerian Duct syndrome (PMDS) is characterised by the presence of Müllerian structures in a 46XY male. PMDS can result from either a defect in Anti-Müllerian hormone (AMH) production or in the AMH receptor. AMH causes the Müllerian ducts to atrophy, enabling the testes to move transabdominally to the deep inguinal rings and into the scrotum. In the absence of AMH action, PMDS can cause problems with testicular descent. We present a f...

Is there a role for medical management in childhood obesity? A review of the Manchester Metabolic Obesity Service

Tan Sherie , Skae Mars , Banerjee Indi , Padidela Raja , Ehtisham Sarah , Mughal Zulf , Clayton Peter , Patel Leena

Background: Childhood obesity is a growing problem worldwide, with serious effects on child health. Obese children are at a higher risk of developing metabolic co-morbidities earlier in life (WHO, 2013). Manchester has worse than national average levels of obesity, with an estimated 14 000 obese children (PHE, 2014).Aims and methods: A retrospective case note analysis of 117 obese paediatric patients, seen in our service between March 2012 and 2014, was ...

An audit to assess the impact of increasing the borderline blood spot TSH cut-off on the detection of cases of congenital hypothyroidism (CHT) identified via newborn screening

Tetlow Lesley , Ehtisham Sarah , Hird Beverly , Manfredonia Claire , Nice Daniella , Scargill Jonathan , Steele Caroline

Background: The UK Newborn Screening Programme Centre Clinical Referral Standards and Guidelines for CHT (2013) define TSH cut-offs for screen positive (>20 mU/l) and borderline (>10 and <20 mU/l) results. In Manchester levels >8 and <20 mU/l are classified as borderline. This audit aims to assess whether adopting the national cut-off would result in babies with significant and permanent hypothyroid disease being missed.Patient popula...

Educating children in continuous subcutaneous insulin infusion (CSII) therapy; are we improving diabetes control?

Lee Alice , Campbell Judith , Marshall Marie , Ainsworth Sue , Salisbury Louise , Bone Mark , Doughty Ian , Ehtisham Sarah

Background: Continuous subcutaneous insulin infusion (CSII) is proving superior in reducing HbA1c compared to multiple daily injections (MDI) in both the adult and paediatric populations. This study aims to compare the two methods, and evaluate the importance of education when starting insulin pump therapy in children.Design: Patients who attended a ‘pump school’ provided by the Royal Manchester Children’s Hospital between January 2010 and...

Octreotide treatment for congenital hyperinsulinism can cause hepatitis

Avatapalle Bindu , Rigby Lindsey , Patel Leena , Ehtisham Sarah , Skae Mars , Padidela Raja , Banerjee Indraneel , Clayton Peter

Introduction: Congenital hyperinsulinism (CHI) is a rare condition of dysregulated insulin secretion causing hypoglycaemia. Oral Diazoxide is used as first line therapy for CHI. In those who are Diazoxide unresponsive, subcutaneous Octreotide is used as second line treatment. Octreotide has recognised side effects of biliary stasis. Additionally, we report hepatitis as a complication of Octreotide therapy in a child with CHI.Case report: A neonate with C...
