Active surveillance of indeterminate thyroid nodules beyond cytological diagnosis, ultrasound evaluation and molecular analysis

Santamorena Graziana , Gay Stefano , Mittica Miranda , Monti Eleonora , Massa Barbara , Giusti Massimo

Most thyroid nodules are benign after fine needle aspiration biopsy (FNAB). Nevertheless approximately 10–25% of nodules are classified in indeterminate classes. Literature reports different risks of malignancy in Thy 3 – Thy 4 nodules. In our center malignancy was observed in 26% of Thy 3a nodules and 14% of the Thy 3f nodules (Giusti et al. 2017) while it was >75% in Thy 4 nodules. Strategies for nodules stratification according to risk are now needed ...

Diagnosis of subacute thyroiditis (SAT) in 2021, during COVID-19 vaccination: observation in a single private secondary endocrine centre in the Savona district, Liguria, Italy

Giusti Massimo , Sidoti Marilena

SAT due to viral infections is a cause of thyrotoxicosis, neck pain and tenderness on palpation. In the COVID-19 era, SAT seems to be a manifestation of the disease which occurs during SARS-CoV-2 infection. In 2021, some SAT cases were reported after COVID-19 vaccination. We reviewed the incidence of SAT from 2000 to 2021. In the Savona district vaccination of the general population started in February 2021 and progressively involved fragile, healthy elderly, adult and young-a...

Liquid vs tablet levo-thyroxine formulation in de novo treatment of hypothyroidism in patients with Hashimoto's thyroiditis: tolerability and changes in quality of life

Sidoti Marilena , Giusti Massimo

Several levo-thyroxine (L-T4) formulations are now available. However, scientific societies discourage the use of generic L-T4 owing to its uncertain bioavailability in comparison with brand L-T4. Recent literature data suggest that using a liquid formulation of brand L-T4 improves the management of hypothyroidism. However, it is not clear whether patient satisfaction improves on liquid (LI) L-T4, as patients may initially...

Short-term effects of thyroid hormone therapy on quality-of-life evaluation by ThyPRO questionnaire in chronic autoimmune thyroiditis

Sidoti Marilena , Giusti Massimo

In chronic autoimmune thyroiditis, increased TPOAb levels have been found to be associated with impaired quality-of-life (QoL). Aim of the study was to evaluate QoL by means of ThyPRO in women with positive TPOAb levels. From January to December 2011, 62 women were invited to the study. Inclusion criteria were: age >18 years, TPOAb >100 mIU/l, no previous therapy for thyroid, no current wish to become pregnant and availability of an E-mail address. Women were randomize...

Benign and malignant thyroid pathology in Albanians living in the Genoa district, Italy: a comparison with data from the Tirana district, Albania

Teliti Marsida , Akshija Ilir , Osmani Migena , Ferone DIego , Giusti Massimo

BackgroundIn Liguria, a region of north-western Italy, the Albanian population is currently estimated to be around 20 000, constituting 15% of foreign citizens living in this area; approximately 50% of these Albanians reside in the Genoa district.MethodsWe retrospectively reviewed the medical records of all the outpatients of the Endocrine Unit of San Martino Polyclinic Hospital from 2002 to 2019. Data on sub...

Anti-Mullerian hormone (AMH) in pre-menopausal females after ablative radioiodine (RAI) treatment for differentiated thyroid cancer (DTC): single-center study

Mittica Miranda , Comite Paola , Campana Claudia , Gay Stefano , Mussap Michele , Giusti Massimo

Background: It is generally reported that female fertility is not affected by high-dose RAI treatment. AMH has been considered a reliable and accurate index of ovarian reserve. There are few data on AMH values in thyroid diseases. Recently, in DTC women who had undergone RAI ablation treatment, Acibucu et al. (2016) reported low AMH levels without significant impairment of fertility.Aim: The aim of this single-center study was to evaluate AMH le...

Adjuvant metformin (M) therapy in differentiated thyroid carcinoma (DTC)

Balestra Margherita , Gay Stefano , Pera Giorgia , Mortara Lorenzo , Monti Eleonora , Giusti Massimo

M shows an antiproliferative effect. In vivo, M reduces TSH in type 2 diabetes mellitus (T2DM) and DTC aggressiveness in DTC with T2DM. The aim of the study was to evaluate in DTC diabetic (Gr1, n=30 under M 1500 mg/day) and non-diabetic without (Gr2, n=83) or under M (gr3, n=84; M 1000 mg/day) the adjuvant role of M. Clinical and laboratory examinations were performed up to 24 months. At baseline the DTC groups were similar for age, gender, tumor st...

Clinical evaluation and outcome of indeterminate (Thy 3) thyroid nodules

Monti Eleonora , Balestra Margherita , Mortara Lorenzo , Gay Stefano , Pera Giorgia , Adorno Alberto , Giusti Massimo

In thyroid nodules with indeterminate cytology, presurgical evaluations for risk management comprise biochemical tests, ultrasonography (US), elastography-US (USE), contrast-enhanced US (CEUS) and mutation analysis. The cytology of 130 Thy 3 nodules was reviewed according to the BTA 2014 classification. Nodules were divided into Thy 3a and Thy 3f categories. Histology was available in 97 nodules. Malignancy was the final diagnosis in 19% of surgically treated nodules. No signi...

The presence of B-RAF V600E and K601E mutations in our Ligurian population

Eleonora Monti , Lorenzo Mortara , Simonetta Zupo , Gianluca Ansaldo , Mauro Truni , Francesco Minuto , Massimo Giusti

Introduction: Differentiated thyroid carcinoma (DTC) is the most common endocrine neoplasm and its rate it’s constantly increasing. Papillary thyroid carcinoma (PTC) represents 87% of all DTC and its incidence is raising at 89%. The thyroid carcinoma preoperative diagnosis consists in fine-needle ago aspiration (FNAB). However in the diagnostic cytology there actually is a ‘grey zone’, the ‘follicular lesion of indeterminate significant or suspicious for fo...

Estimated 10-year risk of bone fracture in women with diffentiated thyroid cancer on TSH-suppressive levo-thyroxine therapy

Vera Lara , Gay Stefano , Campomenosi Claudia , Mortara Lorenzo , Pera Giorgia , Monti Eleonora , Minuto Francesco , Giusti Massimo

After thyroidectomy (Tx) and RAI therapy, patients with diffentiated thyroid cancer are treated with levo-thyroxine to suppress TSH levels. Whether hyperthyroxinemia causes osteoporosis is debated. The aim of this study was to evaluate bone mineral density (BMD) and the fracture risk assessment tool (FRAX) in DTC women. FRAX calculates the 10-year probability of hip fracture (HF) and major osteoporotic fracture (MOF) in subjects aged >40. 46 women with DTC diagnosed and tr...
