Red ears and blue hands in thyroid clinic – anti-thyroid medication causing rare connective tissue diseases

Platts Julia , Gopalappa Jagannath

We present two cases of rare connective tissue disease precipitated by antithyroid medication. The first case is a 37-year-old woman from the Philippines who presented with thyrotoxicosis. She was commenced on Carbimazole, which was subsequently switched to propylthiouracil, as she wanted to become pregnant without delay. Within 2 months she had developed Raynaud’s phenomenon and a facial rash. Autoantibody testing showed a positive ANA of speckled pattern, negative doubl...

Nagarajah kalyani , Panagiotou Grigorios , Karen Platts Julia

Background: Bruns Garlands syndrome is a rare disorder in people living with diabetes, manifesting as unilateral or bilateral muscle pain, weakness and atrophy in the proximal region of the lower limbs. Herein, we present the case of a patient with Bruns-Garland syndrome caused by sodium/glucose co-transporter 2 inhibitors (SGLT2i).Case description: A 55-year-old male with background of type 2 diabetes for 5 years, and currently on oral therapy, presente...

Outcome of radio-active iodine treatment for thyrotoxicosis in Glan Clwyd Hospital 2003–2005

Swidan Ahmed , Platts Julia , Hassanein Mohammed

Background: There is controversy about the ideal outcome for radio-active iodine (RAI) treatment in thyrotoxicosis and the optimal dose to be given. Various centres use various doses ranging from 185 mBq to 550 mBq.Aim: Our aim was to determine the thyroid status post RAI treatment for thyrotoxicosis and in those who became hypothyroid, to assess the time of onset of hypothyroidism from the date of RAI treatment administration.Meth...

Screening in primary care using fasting glucose uncovers significant hypoglycaemia including an asymptomatic insulinoma

Wordsworth S , Hassanein M , Platts J

Contracts with primary care encourage screening for diabetes and as a result a fasting blood glucose is frequently performed on asymptomatic patients. During the last year three such patients were found to have significant hypoglycaemia with elevated c-peptide levels and were referred for further investigation. The first, a 50 year old woman, developed significant hypoglycaemia 22 hours into a fast with elevated c-peptide and insulin levels. A CT scan of ...

This is a case of recurrent muscle paralysis in a patient with both conditions with the underlying abnormality being the neurological condition of hypokalaemic periodic paralysis.A 23-year-old Caucasian gentleman was admitted to medical admissions with sudden onset weakness of his lower legs. On examination he had reduced power of hi...
