Introduction: The diagnostic procedure of primary aldosteronism (PA) includes one of four confirmatory tests, of which the saline suppression test (SST) seems to be the most convenient one. Current guidelines recommend SST to be performed in a recumbent position. However, a recent preliminary study demonstrated a higher sensitivity of SST when performed in the upright (i.e. seated) position. Here, we report a case of posture-responsive PA with a negative recumbent SST but clea...

Screening of thyroid disorders in pregnancy has been a matter of controversy. Recent recommendations favour targeted high-risk case finding though this approach may miss a significant number of those affected (Vaidya et al. JCEM 2007).Among 398 non-selected women in the 10th week of pregnancy, thyrotropin (IRMA) >3.5 mIU/l was found in 10.3%, free thyroxine (RIA) <10 pmol/l in 2% and thyroperoxidase antibodies (RIA) >50 IU/ml in...

Screening of autoimmune thyroid disorders in pregnancy in a highland district

Horacek J , Spitalnikova S , Cepkova J , Ceeova V , Dolezal J , Vizda J , Maly J

On their first antenatal visit (median 10th gestational week), 977 pregnant women in one district were assayed for TSH (IRMA, non-pregnant normal range 0,15-5 mIU/l) and TPO-Ab (RIA). Abnormal values were found in 154 women (15.8%): positive TPO-Ab in 92 (9.4%), TSH<0,15 in 35 (3.6%), TSH>5 in 49 (5.0%). Of them, 128 women were examined in our endocrine clinic (median 15th week), incl. thyroid ultrasonography (7.5 MHz, Doppler), and assayed for TSH, free T4, free T3, a...
