Cinecalcit in the treatment of resistant hypercalcaemia of parathyroid cancer

Theofanoyiannis Panos , Laji Ken

A 64-year-old Caucasian woman presented with hypercalcaemia in 1994 with Parathormone (PTH) of 59.9 pmol/l and corrected Calcium of 2.68 mmol/l.A parathyroid was removed and histology revealed carcinomatous change with vascular invasion suggesting probable dissemination. In November 1998 her corrected Calcium was 2.74 mmol/l with a PTH of 7.2 pmols/l but in 1999 the PTH ranged between 157 and 195 pmol/l with corrected Calcium above 2.80 mmol/l.<p cla...

Two Unusual Cases of Subacute Thyroiditis (SAT) and management

Zheng Dongling , Muralidhara Koteshwara , Theofanoyiannis Panayiotis

Subacute thyroiditis (SAT) is a self-limited inflammatory condition of the thyroid characterised by a clinical course of hyperthyroidism, hypothyroidism, and then return to normal thyroid function. The inflammation is thought to be triggered by a viral infection and is usually treated with anti-inflammatory medications and steroids. Recurrences can uncommonly occur and could be difficult to manage. There are some promising reports of the role of Colchicine in managing recurren...

Kurera Isuri , Ismail Rayan , Xia J , Theofanoyiannis P , Homfray T , Bano Gul

The Fibroblast Growth Factor 23 (FGF23) gene provides instructions for making a protein called FGF23. This protein is necessary in regulating the phosphate levels within the body. We present a case of an adult Caucasian man with persistent hypophosphatemia due to mutation in ENPP1.Case History: A 34 years old male presented with intermittent muscle pain, cramps and headaches. He also noticed episodic breathlessness and irritability. He had no hi...

A case of pseudohyperkalaemia in a patient with myeloproliferative disorder and acute kidney injury

Eyzaguirre Valencia Mario , Tuddenham Emma , Theofanoyiannis Panayiotis , Muralidhara Koteshwara , Chinnasamy Eswari

Introduction: Pseudohyperkalaemia is one of the most common testing errors that occur in clinical practice. It’s commonly due to pre-analytical factors many of which are clinically well recognised. These include incorrect sampling, delay in sample processing and haemolysis to name a few. However non-recognition of some rarer causes like thrombocytosis often results in initial inappropriate, potentially dangerous treatment. Here we present one such case of pseudohyperkalaemia i...

Eyzaguirre Valencia Mario , Chandrashekar Sunaya , Chinnasamy Eswari , Kyaw Ye , Theofanoyiannis Panayiotis , Muralidhara Koteshwara , Sivakumaran Darshi , Ofo Enyinnaya , Yella Ramesh

Introduction: Primary hyperparathyroidism (PHPT) is a commonly encountered endocrine pathology. Asymptomatic renal stones have been reported in 7-22% of patients. NICE guideline (NG132) published in May 2019 on PHPT recommends renal US in all patients. The value of renal ultrasound for all patients with PHPT has been debated. This audit aims to compare local practice to NICE guideline (NG132) and to see if a tailored approach for renal ultrasound would be safe, practical and e...
