Psychiatric disorders in Cushing’s syndrome and impact on follow-up

Introduction: Cushing syndrome (CS) is a rare and serious disease with high mortality. Diagnosis and management of the disease are difficult, as CS is typically characterized by the presence of multiple symptoms hypertension, diabetes, weight gain, or osteoporosis, the psychiatric and neurocognitive consequences are just as important by a set of anxio-depressive disorders resounding on the quality of life but also on the management of the disease.

Objective: Describe the association of Cushing’s syndrome with anxiety and depression disorders and retention on adherence to treatment and follow-up of the disease.

Methods: Descriptive retrospective study of patients hospitalized for endogenous hypercortisimes in the endocrinology and metabolic diseases department between 2018 and 2022.

Results: Our study included 64 patients, the average age was 29.6 years, with a clear female predominance. memory loss with difficulty concentrating was noted in 95%, insomnia with difficulty falling asleep in 80%, depression in 75%, a psychiatric consultation was carried out in the majority of our patients, 50% were put under anxiolytic and antidepressant treatment, only one case of visual hallucination was recorded. As for adherence to treatment and follow-up, 82% showed up regularly for consultation, 6% intermittently and 12% were lost to follow-up.

Conclusion: Psychological management is part of the multidisciplinary management of Cushing’s syndrome. Psychological interventions may be required to treat depression and to improve long-term prognosis, subjective wellbeing and quality of life.
