Post radioiodine monitoring: do patients feel telephone consultations are an acceptable part of a nurse-led clinic?

Rationale for change: Two years ago the Endocrine department instigated an audit of patients who had received radioiodine treatment (RAI) for hyperthyroidism. This highlighted there was a variation in timeliness of follow-up appointments and at least a third of patients were rendered symptomatic as a result of the treatment, prior to review taking place.

Innovation: As a result of the initial audit a nurse-led clinic was established to monitor patients for the first six months after RAI treatment. Patients now have monthly appointments with the Endocrine Nurse Specialist (ENS). These are a mixture of 3 telephone appointments and 3 clinic attendances. A survey of patients attending this clinic showed high levels of satisfaction with this clinic/system.

Survey results: A survey was sent to the first 48 patients who attended the clinic. Twenty-six were returned (54%). When asked what they thought about some of the consultations being carried out over the telephone 100% of those who replied felt this was a good idea. Most commonly they quoted convenience, saving travelling time/costs, and avoiding time away from work. Ninety-six percent felt there were benefits to being monitored monthly and were happy to be followed up be the ENS. Ninety-six percent of patients stated they were very satisfied with the service.

With closer monitoring of thyroid function any additional treatment was initiated earlier, leading to less morbidity for these patients. Patients also welcomed being able to contact the ENS between appointments if they develop thyroid symptoms. Qualifying as an Independent Prescriber has enhanced the running of this clinic.

Conclusion: Patients are happy to participate in telephone consultations with a nurse specialist and value the continuity of one professional overseeing their care post radioactive iodine.
