Insulin treatment of patients with cystic fibrosis and impaired glucose tolerance arrests the decline in pulmonary function

Drummond Russell , Carty David , Small Michael , Jones Gregory

Critical clinical changes of pulmonary function and weight occur in patients with cystic fibrosis (CF) antecedent to the development of frank Diabetes Mellitus (DM). Impaired glucose tolerance (IGT) signifies diminished Insulin secretion and increased peripheral insulin resistance, correlating with worse clinical status, under nutrition and impaired pulmonary function. Insulin therapy has been associated with improvement in anthropometric data and an arrest in decline in force...

Incidence, awareness and apparent symptoms of hypoglycaemia in patients with cystic fibrosis treated with insulin

Drummond Russell , Carty David , Small Michael , Jones Gregory

Diabetes Mellitus (DM) and Impaired Glucose Tolerance (IGT) in patients with cystic fibrosis(CF) signifies a six fold rise in mortality and is associated with declining pulmonary function. The pathophysiology is multifactorial but largely due to Insulinopenia which is due to global islet cell fibrosis and accompanied by loss of islet alpha cells. Insulin treatment improves weight, the bacterial milieu, and lung function but is associated with hypoglycaemia. These patients may ...

Introduction of an intensive outpatient education programme is acceptable to parents of children, and young people with newly diagnosed type 1 Diabetes

Kershaw Melanie , Krone Ruth , Drummond Lesley , Dias Renuka , Barrett Timothy

Objectives: Many successful European centres provide intensive education as 2 week inpatient admissions for newly diagnosed type 1 diabetes. Prolonged inpatient stay is resource intensive and disrupts the family unit. Our centre aimed to determine the feasibility of delivering an intensive education programme in an ambulatory care setting. Methods: The curriculum, introduced in October 2013, comprised 20 hours face to face education by paediatric diabetes...

HENRY: High HbA1c service Evaluation 2016-2019: New “tipping point” in contact Required for CYPwD

Pemberton John , Krone Ruth , Kershaw Melaine , Drummond Lesley , Paget Stephanie , Neylon Pauline

Background: Children and young people (CYPwD) with a persistent high HbA1c are at greater risk of developmental and long-term complications. Diabetes services across the England & Wales are mandated to have a ‘high HbA1c’ policy by the Best Practice Tariff quality standards, to improve health outcomes. Aim: To evaluate the Birmingham Women’s and Children’s (BWC) Diabetes Team ‘High HbA1c Policy’ from 2016 – 2019.</p...

AIP mutation causing familial pituitary tumours

Cordiner Ruth , McManus Frances , Hughes Kate , Panarelli Maurizio , Boyle James , Drummond Russell , Carty David

Familial isolated pituitary adenoma (FIPA) is an increasingly recognised cause of familial pituitary tumours with autosomal dominant inheritance. An increased population risk of AIP mutations has recently been reported in Ireland. We present the cases of three siblings, with likely AIP related disease, attending endocrinology clinics in Glasgow. Patient one has been confirmed to be an AIP mutation carrier.Patient 1 ...

Diasend download data and relation to diabetes control in a tertiary clinic cohort

Muniu Susan , Biss Chloe , Krone Ruth E , Barret Tim , Pemberton John , Drummond Lesley , Kershaw Melanie

Our large tertiary hospital-based diabetes service high HbA1c policy selects Children and Young People (CYP) with HbA1c above 64 mmol/mol for additional support. Two-week average blood glucose (ABG) is utilised in the high HbA1c clinic as a proxy for HbA1c and CYP are encouraged to reduce their 2 week ABG as a primary goal.Aims: To determine the relationship between HbA1c, 2 week and 3 month ABG and standard deviation (S.D.) in CYP with Ty...

IGF2 related non-islet cell tumour hypoglycaemia in a patient with hepatic sarcoma

Ambrose Alice , Butterly Elaine , Drummond Richard , Carty David , Currie Gemma , Boyle James , Hughes Kate , Perry Colin

Hypoglycaemia is a common and potentially life threatening presentation to Emergency Departments across the UK.It is often a result of medications for treatment of diabetes but other differentials include rarer reactive and fasting causes. This case report describes a 69 year old man who initially presented with right upper quadrant pain and weight loss which then lead to a diagnosis of inoperable hepatic sarcoma. He later presented with hypoglycaemia and Whipples triad in the ...

Internal carotid artery haemorrhage in a patient with a radiotherapy treated pituitary macroadenoma with sphenoid extension and osteonecrosis

Butterly EW , Boyle R , Drummond R , Carty DM , Boyle JG , Currie G , Hughes KA

Pituitary macroadenomas often extend to the suprasellar region, however rarely they can extend inferiorly and include erosion into the sphenoid bone, presenting unique challenges. We present a 74-year-old female who received pituitary radiotherapy in 1995 for a pituitary macroadenoma with sphenoid extension. She initially presented in 1994 with secondary amenorrhoea and hyperprolactinaemia (30,000 mu/l). She could not tolerate MRI and subsequent CT showed a pituitary macroaden...
