Unusual presentation of primary hyperparathyroidism

Shillo Pallai Rappai , Balasubramanian Sabapathy

An 80 year old patient was diagnosed with asymptomatic, mild primary hyperparathyroidism in 2005 and managed conservatively with regular biochemical monitoring. Other medical history included atrial fibrillation, hypertension, CKD stage 3, type 2 diabetes and memory impairment. In 2011, in view of increasing calcium, he was referred to the endocrine surgeons to consider parathyroidectomy. A very low 25 (OH)vit D (12 nmol/L) prompted high dose oral cholecalciferol treatment. Th...

We should “ruddy well follow them up” - An unusual complication of suboptimally treated Congenital Adrenal Hyperplasia

Shillo Pallai Rappai , Macinerney Ruth , Robinson Robert

A 40 year old man with CAH and significant needle phobia presented with left leg pain in January 2011. He had been discharged from follow up at another trust. Medications were hydrocortisone 10 mg BD and fludrocortisone 100 mcg OD. Admission haemoglobin was 19.9 g/dl, haematocrit 0.554. A thorough work up to investigate causes of polycythaemia revealed the only significant abnormality as a testosterone of 45.7 nmol/L (8–27), with low gonadotrophins. The blood sample for 1...

Hypercalcaemia: a mixed family picture

Hunt Leanne , Robinson Robert , Shillo Pallia , Cranston Treena , Thakker Rajesh

Hereditary hypercalcaemia can be a diagnostic challenge. We present a family with autosomal dominant hypercalcaemia that illustrates this. A 48-year-old man was referred with asymptomatic long standing hypercalcaemia (2.8–3.04 mmol/l), with plasma PTH levels between 45 and 48 ng/l (normal 15–65), and a high urinary calcium excretion, (24 h calcium collection 10.4 mmol/l (2.5–7.5), with high calcium excretion indexes) all consistent with primary hyperparathyroidi...

Pituitary apoplexy with a twist

Elamin Aisha , Pallai Shillo , Macinerney Ruth , Khalifa Mai , Went Richard

Introduction: Multiple myeloma (MM) is a haematological disorder characterised by an aberrant rise in monoclonal paraprotein. Intrasellar plasmacytomas can be infrequent MM manifestations. They are challenging to diagnose and frequently mimic pituitary adenomas; however rarely present with pituitary apoplexy. We describe a patient with pituitary apoplexy-like symptoms, subsequently diagnosed with MM, whose intrasellar mass entirely resolved after chemotherapy....

Severe Cushing’s Syndrome in a 24 year old female

Sloan Gordon , Macinerney Ruth , Pillai Anita , Robinson Robert , Pallai Shillo , Shahbuddin Ibrahim

A 24 year old female presented to her general practitioner with general malaise. She had a genetic diagnosis of hereditary non-polyposis colorectal cancer (Lynch syndrome), with no colonic lesions detected on screening. Routine biochemistry revealed a potassium of 2.1mmol/l, requiring admission for urgent investigation and intravenous replacement. Further clinical assessment found a past history of severe acute thoracic back pain (3-months prior to presentation), weight gain a...

Radioidine (RAI) as a new therapy for the treatment of ovarian cancer through the sodium/iodide symporter (NIS)

Mirela Mielu Lidia , Paz Lopez-Molina Maria , Mingo-Casas Patricia , Rodal-Bravo Lucía , Diego-Hernandez Cristina , Pallarés Pilar , García-Jimenez Custodia , Martin-Duque Pilar , De la Vieja Antonio

Introduction: Ovarian cancer is the most lethal gynecological malignancy. Early diagnosis has a survival rate of 90%. Unfortunately, more than 70% of cases are diagnosed when the cancer has already metastasized, and survival rates do not exceed 30% in these cases. The sodium iodide symporter (NIS) is an integral plasma membrane glycoprotein expressed in the basolateral surface of the thyroid gland, where it mediates active transport of iodide. Radioiodide therapy (RAI) through...

The use of different sodium/potassium perchlorate regimes could affect response time in the treatment of amiodarone-induced thyrotoxicosis

Pazos Mario , Cuesta Martín , Pallarés Raquel , Ramos Elvira , Barrio Elvira , Puebla Virginia , De Miguel Paz , Ángel Díaz Jose , Calle Alfonso , Runkle Isabelle

Introduction: In the treatment of amiodarone-induced type I or mixed thyrotoxicosis, sodium or potassium perchlorate is often associated to thionamides, when the latter are insufficient for a prompt control of thyroid hormone secretion. Perchlorate competes with iodine for entrance into the follicular cell, and also induces a discharge of iodine from these cells with each administration. We compare the effect of 3 different regimes of perchlorate with methimazole on control of...

Use of alfacalcidol and teriparatide for postsurgical hypoparathyroidism in a patient with gastrostomy and toxicity after systemic antineoplastic therapy: A case report

Barrio Elvira , Ramos Elvira , Marcuello Clara , Pazos Mario , Pallarés Raquel , López Celia , Pérez Natalia , Cabezas Santiago , Ángel Rubio Miguel , Matía Pilar

Introduction: Post-surgical hypoparathyroidism (PSHP) is a frequent complication following ENT surgery. Some patients with head/neck cancers require enteral nutrition (EN) due to dysphagia, mucositis after radiotherapy or chemotherapy, or post-surgical anatomical complications. Formulas of calcitriol are not suitable to be administered via enteral feeding tubes, and side effects of systemic antineoplastic treatment (nausea, vomiting) make calcium and phosphate serum levels dif...

Importance of lymph extranodal extension to therapy outcomes in patients with thyroid follicular epithelial cell-derived carcinoma evaluated at one year

Fernando Hernández Olmeda , Patricia Espinosa De Los Monteros , Xavier Pérez Candel , Lopez Nevado Celia , Raquel Pallarés , Inés Jiménez , Maria Cristina Familiar Casado

IntroductionThe number of lymph node metastasis (NM) and the existence of extranodal extensión (ExEx) are not included among prognosis variables used in the most common staging systems for differentiated thyroid cancer as AJCC and ATA risk systems.ObjectiveIn patients with NM at diagnosis, to analyze the association between the number of NM and the presence of ExEx with response to therapy.<p class="...
