Familial SDHC mutation associated with prolactin/gh-secreting pituitary adenoma and paraganglioma

Barigou Mohammed , Buffet Alexandre , Bennet Antoine , Pigny Pascal , Bellec Laurent , Caron Philippe , Vezzosi Delphine

Introduction: SDH genes mutations are associated with hereditary phaeochromocytoma and paraganglioma syndromes. We describe the case of a patient with SDHC related familial paraganglioma and pituitary adenoma.Case: A 65-year old man consulted for an incidentally discovered 7 cm abdominal mass on CT-scan, lateral to the right kidney, invading inferior vena cava, associated to a retroperitoneal adenomegaly and a lesion on the body of L2 vertebra with spina...

The patient with broken heart

Solomon Alexandra Lubina , Iqbal Ahmed , Harrison Barney , Webster Jonathan , Bennet William

A 58-year-old gentlemen presented with severe neckache, sweating, feeling hot, and cold. PMH included hypertension and MI. On examination, he was clammy, with blood pressure (BP) 205/107 mmHg. Laboratory results showed leucocytosis 28.5 and TroponinT 2002 ng/l (reference range <14.0). ECG showed inferolateral ST depression. CT angiogram showed a normal aorta and a 6 cm right adrenal mass. An echocardiogram showed severe LV impairment and normal valves.<p class="abstext...

Maternal and foetal outcomes associated with medical and surgical treatment of primary hyperparathyroidism in Pregnancy

Fahad Arshad Muhammad , Arambewela Maulee , Bennet William , Sterrenburg Monique , Balasubramanian Saba P

Background: The management of Primary hyperparathyroidism (PHPT) during pregnancy is challenging due to lack of robust guidelines. Various studies have suggested an increased risk of various maternal and foetal complications, however there is no clear consensus in the literature about medical treatment vs surgery during pregnancy. The aim of this study was to review the maternal and foetal outcomes of pregnant women treated for PHPT at a single tertiary care centre and compare...

Decreased IGF1 levels and GH-resistant hepatic state to estrogens during the first trimester of pregnancy in non-acromegalic women

Persechini M. , Gennero I. , Grunenwald S. , Vezzosi D. , Bennet A. , Caron P.

Introduction: In women with GH-secreting pituitary adenoma, a decrease of IGF1 levels has been reported during the first trimester of pregnancy, before placental GH (pGH) secretion inducing a progressive increase in IGF1 levels throughout gestation. This decrease has been related to hepatic GH-resistant state, via JAK2/STAT pathway, due to increased estrogen (E2) levels. Objectives: Evaluate IGF1 change in non-acromegalic women during the first...

Diagnosis of post-surgical recurrence in patients with malignant insulinoma

Buffet A , Vezzosi D , Maiza J-C , Grunenwald S , Bennet A , Otal Ph , Carrere N , Caron Ph

Context: The post-surgical follow-up of malignant insulinomas is often marked by recurrence after initial remission. However, the criteria for the diagnosis of recurrence of these malignant endocrine tumors are not well defined in the literature, unlike diagnostic of criteria for hyperinsulinemic hypoglycaemia (Cryer 2009). We retrospectively reviewed the criteria that allowed the diagnosis of recurrence in four patients with malignant insulinoma after initial post-operative r...

Retinol binding protein 4 suppresses during normal pregnancy – an inverse relationship with insulin resistance

Pierides Michael , Lewandowski Krzysztof , Bennet Jeanette , Dunlop Diane , Randeva Harpal , O’Hare Paul

Retinol binding protein-4 (RBP4) is an adipokine reportedly linked to physiological states of insulin resistance and impaired glycaemia. Pregnancy is a physiological state characterised by changes in carbohydrate metabolism and increased insulin resistance. Our aim was to determine plasma levels of RBP4 during early pregnancy, late pregnancy and postpartum in normal women.Fasting plasma levels of RBP4, insulin and glucose were collected and assayed (ELIS...

Treatment of severe symptomatic hyponatraemia using hypertonic saline; real world findings from 2017–2020 in two university hospitals in the UK

Fahad Arshad Muhammad , Iqbal Ahmed , Weeks James , Fonseca Ines , Munir Alia , Bennet William

Introduction & aimHypertonic saline (HTS) is an effective treatment for severe symptomatic hyponatraemia. European Society of Endocrinology (ESE) published guidelines to standardise HTS administration in 2014, however, evidence behind the guidelines is limited by virtue of being largely derived from small retrospective studies and expert opinion. The aim of this study was to evaluate the ’real-world’ safety and efficacy of these guidelines....

Case series of catecholamine-induced Takotsubo cardiomyopathy secondary to pheochromocytoma and discussion of relevant literature

Lin Zhimin , Soh Bennet , Lee James , Yuan Ngiam Kee , Parameswaran Rajeev

Catecholamine-induced cardiomyopathy is a rare presentation of pheochromocytoma (PCC) with typical and atypical subtypes. These patients are associated with worse rates of morbidity and mortality compared to patients with PCC that were incidentally found. Frequently, these patients would present in the emergency setting with cardiac symptoms that may be difficult to distinguish from ischemic cardiomyopathy, and hence patients may be prescribed cardiac treatment in the form of ...

Association of variants within the fat mass and obesity-associated (FTO) gene and polycystic ovary syndrome

Barber Thomas , Bennett Amanda , Groves Christopher , Sovio Ulla , Ruokonen Aimo , Martikainen Hannu , Pouta Anneli , Hartikainen Anna-Liisa , Elliott Paul , Lindgren Cecilia , Freathy Rachel , Koch Kirsten , Ouwehand Willem , Karpe Fredrik , Conway Gerard , Wass John , Jarvelin Marjo-Riitta , Franks Stephen , McCarthy Mark

Background: Obesity plays an important role in the aetiology of polycystic ovary syndrome (PCOS). Variants in the fat-mass and obesity-associated (FTO) gene have recently been shown to influence adiposity and predispose to common obesity (Frayling et al., Science 2007 316 889–894).Aim: Our aim was to establish whether FTO variants are also implicated in PCOS-susceptibility.Methods: A genetic ...

Clinical case: 24-year-old student nurse referred to the liver unit with a 6-month history of pyrexia, cognitive dysfunction and abnormal liver function tests. ERCP was normal; CT scan suggested multiple liver deposits; histology showed both fatty infiltration and focal cholangitis. Medical history included type 1 diabetes since age 7yrs; an unusual skin rash that spontaneously remitted 4 years previously; recurrent vulval ulceration, variously ascribed to candida and/or excor...
