Complications of phaeochromocytoma: a case of catecholamine-induced cardiomyopathy

Mulholland Sarah , Martin Christopher

Case History: We present a case of a 33-year-old woman who attended with heart failure whilst awaiting a surgical resection of phaeochromocytoma. She presented hours after an intravenous iron transfusion to treat her known iron-deficiency anaemia secondary to menorrhagia from massive uterine fibroids, for which she under the care of the gynaecology team. She was known to have a right-sided phaeochromocytoma, which was found incidentally on MRI in 2021, and was taking phenoxybe...

A rare case of Hyperparathyroidism Jaw Tumour Syndrome without jaw tumours

Mulholland Sarah , Lewis Alex

Case History: A 19-year-old female presented with painful left shoulder after slipping whilst doing press-ups. Plain x-ray showed a proximal humeral fracture, as well as a large bone cyst raising concerns of a pathological fracture. She had no past medical history and was not taking any regular medications. On further questioning she did complain of a 2 year history of bilateral painful shoulders and left knee pain. Family history was significant for her father suffering prima...

Incidence of complications of acromegaly 10 years after achieving biochemical control of GH – a single centre retrospective observational study

Mulholland Sarah , Clayton Richard , Jose Biju , Varadhan Lakshminarayanan

Aim: Acromegaly is associated with multiple co-morbidities including diabetes mellitus (DM), obstructive sleep apnoea (OSA), arthropathy, cardiovascular events and malignancies. Early diagnosis and treatment, with an intention to cure or control the growth hormone (GH) exposure can reduce risk of these complications. The aim of this study is to assess the development of acromegaly-related complications amongst patients who have completed at least 10 years of follow up after ac...

Sex specific differences in overall survival in patients with pulmonary neuroendocrine neoplasms (NENs) in a large United Kingdom (UK) tertiary centre

Clement Dominique , Brown Sarah , Mulholland Nicola , Sarker Debashis , Giwa Mojisola , Koundouraki Antonia , Ramage John , Srirajaskanthan Raj

Introduction: Pulmonary NENs account for around 2% of all pulmonary malignancy. They are classified histologically into 4 types typical and atypical carcinoids which are generally well differentiated and large cell neuroendocrine carcinoma (LCNEC) and small cell lung carcinoma (SCLC) which are poorly differentiated. Within the literature there are several reports regarding overall survival on small mainly surgical cohorts, but data for the UK population are lacking. Only a few...

Carcinoid heart surgery: Review of outcomes from a single large centre

Maclean Rory , Cope Jack , Hota Shweta , Mulholland Nicola , Ramage John , Wendler Olaf , Srirajaskanthan Raj

Introduction: Up to half of patients with carcinoid syndrome develop carcinoid heart disease; this is thought to be mediated by serotonin from disordered tryptophan metabolism. Valve degeneration can lead to right heart failure which drives morbidity and mortality in this patient population. Surgical replacement of affected valves is an effective therapy. Peptide receptor radiotargeted therapy (PRRT) has demonstrated better progression-free survival compared to high dose sando...

Timing of peptide receptor radiotargeted therapy in relation to cardiac valve surgery for carcinoid heart disease in patients with neuroendocrine metastases and cardiac syndrome: a single centre study from a centre of excellence

Davis LM , Nicou N , Martin W , Corcoran B , Mulholland N , Srirajaskanthan R , Ramage J , Wendler O , Vivian G

Introduction: Perioperative mortality of patients who undergo heart valve surgery for carcinoid heart valve disease (CHVD) has been observed to be high (5–10%). We investigated whether peptide receptor radiotherapy (PRRT) with Lutetium-177 Dotatate can be used safely in patients with neuroendocrine neoplasms (NEN) and CHVD and if there is an associated survival advantage by reducing overall exposure of the valves to high doses of vasoactive peptides.<p class="abstext"...

The incidence and clinical significance of metabolically active brown adipose tissue in patients with pheochromocytomas and paragangliomas: A retrospective cohort study, systematic review and meta-analysis

Onyema Michael , Ostarijas Eduard , Minhas Raisa , Roy Aparajita , Kearney Jessica , Omran Asma , Zair Zoulikha , Reynolds Saira , Mulholland Nicola , Corcoran Benjamin , Halim Mohammad , Aylwin Simon , Dimitriadis Georgios K

There is limited research into the impact of active brown adipose tissue (aBAT) in patients with phaeochromocytomas and paragangliomas (PPGLs). A small body of evidence has shown that patients with PPGL patients can exhibit a high prevalence for aBAT recognized as ranging between 8 - 28% without specific correlation to germline mutations. Furthermore, it has been suggested that aBAT may be linked to increased mortality. Systematic searches of the Medical Literature Analysis an...

Implementation of a new adrenal venous sampling (AVS) protocol for primary aldosteronism: early data from a single tertiary centre

Green Deirdre , Farnan Vanessa , McDonnell Tara , Abdelfadil Sabah , Heverin Karen , Carthy Claire , Srinivasan Shari , Hill Arnold , Dugal Neal , Morrin Martina , Dunne Ruth , Mulholland Douglas , Agha Amar , Sherlock Mark , O'Reilly Michael

Primary aldosteronism (PA) is the most common cause of endocrine hypertension. Following biochemical confirmation of aldosterone excess, adrenal venous sampling (AVS) is the gold standard investigation to lateralise the source of aldosterone hypersecretion in patients deemed suitable for potential adrenalectomy. AVS has been performed for all suitable patients with a diagnosis of PA in Beaumont Hospital since implementation of a new protocol involving collaboration between End...
