Recurrent pregnancy induced pituitary apoplexy in a patient with a macro-prolactinoma

Golding Jonathan , Kirresh Omar , Canagon Sara , Sinclair Jo , Wheatley Trevor

A 23-year-old woman was referred with light periods and subfertility with a raised prolactin (1175 miu/l). Remaining pituitary function tests were normal. As she was now pregnant, investigations were postponed until post-pregnancy. Post-partum prolactin was 1823 miu/l and MRI pituitary demonstrated a 13.5 mm pituitary macroadenoma. Cabergoline was commenced but stopped due to a further pregnancy. At 22 weeks gestation she experienced left visual field loss. MRI showed...

A pituitary problem in pregnancy

Voss Samantha , Wheatley Trevor , Canagon Sara , Crown Anna

A 29 year old woman, 21 weeks pregnant with no previous medical history was admitted with 5 days blurred vision in the left eye and headaches, worse in the morning and lying flat. Three weeks previously she had been treated with antibiotics for a group A strep infection. An urgent MRI suggested pituitary apoplexy with left optic chiasm nerve compression. IV hydrocortisone 100 mg TDS was commenced reduced to 20 mg TDS. Investigations showed TSH 0.2 mu/l, FT4 10.4 pmol/l, FT3 2....

Papillary thyroid carcinoma and Graves disease: a case report

Pinheiro Sara Lomelino , Silva Tiago , Pereira Conceicao , Leite Valeriano

Background: Papillary thyroid cancer (PTC) is the most common type of thyroid carcinoma, but it rarely presents as an intrathoracic mass with concurrent hyperthyroidism due to Graves disease.Case presentation: A 73-year-old man presented with weight loss. Imaging by computed tomography (CT) documented a large mediastinal mass in the infrathyroidal space separate from the thyroid gland and pulmonary lesions suggestive of metastases. Neck ultrasound showed...

A case of a TSH-secreting pituitary macroadenoma in a previously well thirty year old woman

Golding Jonathan , Canagon Sara , Norris John , Chakera Ali

Case History: A thirty year old female presented to hospital after a first seizure. She had no significant past medical history. She had a one week history of recurrent frontal headaches and had noticed irregular periods. There were no clinical features of acromegaly. CT and MRI brain imaging showed a 5 cm mass arising from the pituitary fossa, extending into the suprasellar cistern and anterior cranial fossa. There was evidence of mass effect on the optic nerves and optic chi...

Severe hypocalcemia: when the skin expresses itself

Askaoui Sara , Lmghari Guizlane , Rafi Sana , Ansari Nawal El

Introduction: Impetigo herpetiformis is a rare dermatological disorder, mimicking generalized pustular psoriasis. It is characterized by a generalized erythema-pustular eruption accompanied by fever. Its etiopathogenesis is poorly coded but triggers have been identified such as hypocalcemia in more than half of cases and pregnancy. We report a case of severe hypocalcemia secondary to hypoparathyroidism revealed by impetigo herpetiformis.Case report: Fort...

Metabolic profile and diabetes in prison

Askaoui Sara , Oukit Loubna , Lmghari Guizlane , El Ansari Nawal

Introduction: The prison environment is a space that involves certain restrictions on prisoners. The diabetic subject requires, however, special attention. The aim of this work is to evaluate the impact of this medium on the glycemic balance and the metabolic profile.Patients and method: Descriptive cross-sectional study carried out on a health campaign day in a penitentiary center, in the men’s sector, and concerned patients with known diabetes fro...

Mammosomatotropic adenoma and acromegaly: about 3 cases

Askaoui Sara , El Mghari Guizlane , Raiss Hanane , El Ansari Nawal

Introduction: Somatotropic adenomas are clinically expressed either by acromegaly or by gigantism according to age of onset. Several histological types are involved. Immunohistochemistry provides conclusive evidence that significant diversity exists between growth hormone secreting (GH) tumors in excess. We report three observations of a particular histological type: mammosomatotropic adenoma.Observation 1: Mrs. M.H., aged 56, hypertensive for 10 years, ...

Silent somatotropic adenoma: about a case

Askaoui Sara , Lmghari Guizlane , Rafi Sana , El Ansari Nawal

Introduction: Somatotropic adenomas are classically a source of hypersecretion of GH and consequently of IGF1 responsible for the clinical signs of acromegaly. Rarely, these adenomas remain ‘silent’, without any obvious clinical manifestation. They can then be detected on the basis of a routine determination of growth hormone under oral glucose tolerance test (GH/OGTT) and insulin growth factor 1 (IGF1) or be diagnosed only to immunohistochemical study as is the case...

Thyroid angiosarcoma: in the light of a case

Askaoui Sara , Lmghari Guizlane , Rafi Sana , Ansari Nawal El

Introduction: Angiosarcoma is a highly malignant tumor, very rare and even more so when it comes to a thyroid localization. It is characterized morphologically by its vascular differentiation and cytological polymorphism. The objective of this study is to report a case of this entity with its evolution in the postoperative period.Observation: patient of 54-year-oldm, with no pathological history. Operated (total thyroidectomy) for compressive multihetero...

Therapeutic education specifically dedicated to patients being followed for stunting with somatotropin

Askaoui Sara , Rafi Sana , Lmghari Guizlane , Ansari Nawal El

Introduction: Therapeutic education is one of the cornerstones in the management of various chronic diseases in children and adults. Several models of therapeutic education have been well developed (obesity, type 1 diabetes…); nevertheless, it remains less common and less developed in certain endocrine diseases. The purpose of this work is to present the therapeutic approach designed for children requiring treatment with growth hormone.Materials an...
