An unusual presentation of hypopituitarism

Boyle J , Deans K , Hopkinson H

A 43 year old man presented following a seizure. Past history included cranial irradiation 18 years ago for nasopharyngeal carcinoma (NPC). Blood pressure was 100/66 with a postural drop. Examination revealed a bilateral sixth nerve palsy. The only biochemical abnormality on admission was a serum sodium of 117 millimoles per litre. A cranial CT scan was normal.Further investigation included thyroid stimulating hormone of 3.92 milliunits per litre, a free T4 of 4 picomoles ...

Possible Functional Regression of Insulinoma with Prolonged Octreotide

Craig C , Gallen I

A 75 year old woman was treated for over three years with the somatostatin analogue, octreotide for insulinoma. She had presented in a hypoglycaemic coma. C-peptide and insulin levels were both raised. Cortisol, TSH, a gut hormone screen and urinary sulphonylurea levels were all normal. No adenoma was identified on CT or MRI scanning but angiography showed an area of increased vascularity within the pancreas. At laparotomy no lesion was found and no resection was undertaken. B...

Effects of fetal adrenalectomy on the binucleate cell population in ovine placenta near term

Ward J , Wooding F , Fowden A

In ovine placenta, binucleate cells (BNCs) are formed from trophectodermal uninucleate cells and produce placental lactogen (PL) and pregnancy associated glycoproteins (PAGs). BNC numbers cells are stable throughout most of pregnancy, yet fall abruptly just before term (Wooding et al., 1994), at a time when endogenous fetal cortisol levels are rising. This study looked at the effect of abolishing the normal prepartum cortisol rise by fetal adrenalectomy on the BNC population i...

A case of adrenal tuberculosis mimicking non-functioning adrenal incidentaloma

Hirani Dhruti , Ladha Tasneem , Khan Shaila , Hilal Zaib , Palazzo Fausto , Vakilgilani Tannaz

A 77 year old gentleman was referred to the endocrinology team following the incidental finding of an adrenal nodule on computer tomography (CT) colonography. Following this, dedicated CT of the adrenal showed a 4cm nodule with an attenuation of 30 Hounsfield Units. Biochemical investigation found no evidence of adrenal insufficiency or functional hormone production. Subsequent imaging by magnetic resonance imaging (MRI) and further CT showed stable appearances in size of the ...

Brown tumors: the first and the final manifestation of primary hyperparathyroidism.

Sztembis Joanna , Orlowska-Florek Renata , Dryja Agata , Swider Grzegorz

Introduction: Brown tumor–well circumscribed lytic lesion is one of the complication of advanced stage of primary and secondary hyperparathyroidism. It may be also a first clinical sign of primary hyperparathyroidism. Nowadays this is rare manifestation in developed countries due to increased use of routine screening laboratory examinations. Brown tumors may also mimic true neoplasm and lead to misdiagnosis if close attention is not paid. We present three case reports and...

How do medical students and doctors learn clinical endocrinology?

Patel Mehul , Druce Maralyn

Background: There has been little research into methods of learning clinical endocrinology, a speciality encompassing rare conditions and pattern recognition. Problems include limited patient exposure outside specialist centres and increasing pressure on doctors to manage their own educational needs. An understanding of learning methods in use across the spectrum of students and doctors will help to identify trusted resources and untapped techniques.Aim:...
