Intrathyroidal thymic carcinoma (ITTC) is a rare thyroid malignancy. Its histology resembles that of squamous cell carcinoma (SCC) of the thyroid. However, ITTC shows a much more favorable prognosis than SCC. We analyzed the backgrounds and clinicopathological features of 10 patients who treated with ITTC in our medical center. Five patients was male, and another five was female. The average tumor size was 4mm, and it ranged from maximum 68mm to minimum 13mm. In pre-operative ...

Introduction: First-generation SRL (fg-SRL) represent the treatment of choice in acromegaly patients with post-neurosurgical adenomatous remnant and GH-hypersecretion. Anyway, approximately 60% of patients do not achieve adequate disease control. The main predictors of resistance include male sex, young age, invasiveness of the lesion and its hyperintensity on T2-weighted MRI scans; but also, SSTR2 expression, the cytokeratin pattern, Ki-67 and the presence of AIP gene mutatio...

Objective: Polycystic Ovary Syndrome (PCOS) is one of the most common endocrine conditions affecting women of reproductive age. Current estimates place the condition as more common than Type 2 Diabetes Mellitus (T2DM). Recent studies have shown several long term comorbidities associated with PCOS, thus making it essential that all physicians, regardless of training and speciality, understand and empower women with PCOS to adopt evidence-based behavioural changes. This study se...

Introduction: Insulinoma, despite its low incidence of 0.4%, it is the most common functioning pancreatic neuroendocrine tumor. Most are benign, solitary and sporadic. Around 10% can be malignant and 5-10% are part of MEN-1. On certain occasions, the differential diagnosis is difficult when there is a history of Diabetes Mellitus (DM). Our objective was to describe clinical-epidemiological data and its diagnostic-therapeutic management.Materials and Meth...

Preference of acromegaly patients for treatment attributes in Spain

Fajardo-Montanana Carmen , Alvarez Escola Cristina , Biagetti Betina , Centeno Rogelio Garci a , Ciriza Raquel , Sanchez Laura , Diaz Marcos

Background: Acromegaly is a slowly progressive rare disease caused by an increase in growth hormone secretion that causes a subsequent rise in insulin-like growth factor (IGF-1), both contributing to the excessive growth of the extremities, soft tissues and organs, in addition to other comorbidities directly interfering with patient’s quality of life. Acromegaly patients are concerned about their disease and their treatments, however, publications about patient’s opi...

Background: Despite global guidelines recommending initiation of SGLT2i drugs like dapagliflozin in type 2 diabetes (T2DM) patients with cardiovascular (CV) or renal risk, its clinical translation is still lacking in India. Objective: To understand clinicians’ perspectives regarding the association of CV risk and T2DM in Indian patients, and the need for the use of combination therapies with dapagliflozin in T2DM patients, in Indian clinical practice...

The effectiveness of different treatment regimens in patients with Graves’ ophthalmopathy

Daria Fomina

According to the recommendations of the European Group for the study of Graves ‘orbitopathy (EOP) (EUGOGO), pulse therapy with glucocorticosteroids, is the method of choice in active EOP of moderate severity or severe course. The choice of scheme depends largely on the doctor’s preferences, as the comparative effectiveness of many of them remains uncertain. Objective: To evaluate the effectiveness of various schemes of pulse therapy with methylp...

Introduction: Dopamine agonists (DA), reference treatment for microprolactinomas, rarely allow long term remission. The performance of endoscopic pituitary surgery should be examined as first-line treatment. Objective: To evaluate the remission rate of microprolactinoma operated by endoscopic transphenoidal approachMethods: We retrospectively studied the surgical results of the 35 microprolactinomas operated between 2010 to 2017 in ...

Blood glucose management

Birkeland Kare

Glucose lowering treatment in patients with type 2 diabetes aims at reducing hyperglycaemic symptoms, preventing acute hyperglycaemic crisis and delaying the onset and progression of microvascular complications. The impact of glucose control on cardiovascular complications remains uncertain, however, a modest benefit is likely to be present. The cornerstones of treatment remain education, motivation and counseling on healthy eating behavior and moderate p...

Alhambra-Exposito Maria Rosa , Lombardo-Galera Maria Sagrario , Ibanez-Costa Alejandro , Rivero-Cortes Esther , Gahete Manuel , Luque-Huertas Raul , Galvez-Moreno Maria Angeles , Castano-Pastor Justo

Acromegaly is caused by excessive GH secretion from pituitary adenomas. Transphenoidal surgery is the first-choice treatment, but new drug therapies (e.g. somatostatin analogues, SSA) offer promising avenues for medical treatment. Complementary diagnostic tools may assist this strategy, helping to refine drug choice. Here, we investigate the associations between radiological features and molecular phenotype of pituitary tumours from acromegalic patients. This observational stu...
