Cinecalcit in the treatment of resistant hypercalcaemia of parathyroid cancer

Theofanoyiannis Panos , Laji Ken

A 64-year-old Caucasian woman presented with hypercalcaemia in 1994 with Parathormone (PTH) of 59.9 pmol/l and corrected Calcium of 2.68 mmol/l.A parathyroid was removed and histology revealed carcinomatous change with vascular invasion suggesting probable dissemination. In November 1998 her corrected Calcium was 2.74 mmol/l with a PTH of 7.2 pmols/l but in 1999 the PTH ranged between 157 and 195 pmol/l with corrected Calcium above 2.80 mmol/l.<p cla...

Hydrocortisone replacement therapy- the controversy continues…

Freel M , Thomson A , Devers M , Campbell K , Grant D , Wallace M , Connell J

Although hydrocortisone is the standard replacement therapy for primary or secondary adrenal insufficiency (AI), there are limited data on dosage requirements and the value of salivary monitoring in this patient group. We assessed inter-individual variability in cortisol metabolism and relationships between plasma and salivary cortisol profiles in 27 patients. Ten (3 male) patients had primary AI and 17 (8 male) secondary AI. Salivary and plasma cortisol levels were measured a...

A more rigorous laboratory definition of macroprolactinaemia with clinical significance

Smith T , Gibney J , Kavanagh L , Dickinson A , McKenna T

The importance of the differentiation between the apparent benign clinical condition of macroprolactinaemia and that of true hyperprolactinaemia, which requires therapy, is becoming widely recognised. Laboratories routinely rely on prolactin recoveries of less than 40 percent following treatment of sera with polyethylene glycol (PEG) to distinguish between the former and latter conditions. However, the 40 percent threshold employed is arbitrarily defined with little scientific...

INSULIN-LIKE GROWTH FACTORS AND BINDING PROTEINS IN HUMAN EJACULATES

Nikolic J , Nedic O , Mihailovic N , Baricevic I

While previous studies have shown relatively low concentrations of insulin-like growth factors (IGF)in human ejaculates, high levels of IGF binding protein-2 (IGFBP-2)have been reported. The IGF/IGFBP system was investigated in ejaculates from thirty men aged 23-46 years consulting a fertility clinic. Volume, viscosity, total and motile sperm counts were estimated by standard methods and each subject consented for the specimen to be used in this study. Local Ethical Committee ...

US-guided percutaneous microwave ablation for benign thyroid nodules: a prospective multicenter study

Liang Ping

Purpose: To evaluate the clinical outcomes of percutaneous MWA in treating BTNs under US guidance with a prospective multicenter study.Materials and methods: From January of 2013 to December of 2015, the total number of 603 patients with 664 benign thyroid nodules (BTNs) at four participating institutions in China was enrolled in the multicenter study. Before ablation, the mean maximal diameter and the mean volume of the target nodules were 2.92±0.9...

Comparison of quality of life and symptoms of depression in diabetes combined with gastroparesis and without it

Orlova Marina , Frolova Arina , Rodionova Tatiana

Objective: To assess the quality of life, the presence and severity of depressive symptoms in patients with diabetes combined with gastroparesis and without itMethods: Forty three patients with diabetes were included in the study. The presence and severity of gastrointestinal symptoms, as well as the intensity and frequency of symptoms over the last 2 weeks, were measured with the Patient Assessment of the upper gastrointestinal symptom severity index (P...

Genetic profiles of aggressive variants of papillary thyroid carcinomas

Jin Meihua , Song Dong Eun , Ahn Jonghwa , Song Eyun , Yu-Mi Lee , Tae-Yon Sung , Kim Tae Yong , Kim Won Bae , Shong Young Kee , Jeon Min Ji , Gu Kim Won

BackgroundAggressive variants of papillary thyroid carcinoma (PTC) have been described with increasing frequency and are associated with unfavorable clinical outcomes. However, limited data exists on the comprehensive genetic profile of these variants.MethodWe performed targeted next generation sequencing in 36 patients with aggressive variants of PTC and compared it to PTC from The Cancer Genome Atlas projec...

Viral hypercalcemia

Latif Wajeeha , Mansoor Raza Mohammed , Ali Asif

A 64-year-old woman presented acutely with dry cough, difficulty in eating, diarrhoea and weight loss. Her past medical history included cervical carcinoma in situ treated with laser therapy. She was found to be hypercalcaemic, with a corrected calcium of 2.84 mmol/l. During admission, the calcium ranged between 2.8-3.87 mmol/l. The PTH ranged between 0.2-0.4 pmol/l (1.3-9.3 pmol/l). She had raised inflammatory markers and deranged liver function tests. On review, she...

Harmonization status among ten European laboratories using liquid chromatography-tandem mass spectrometry (LC-MS/MS) for steroid measurements in serum: preliminary results from the HarmoSter consortium

Fanelli Flaminia , Mezzullo Marco , Temchenko Anastasia , Cantu Marco , Lindner Johanna M , Peitzsch Mirko , Hawley James M , Bruce Stephen , Zelzer Sieglinde , Herrmann Markus , Heijboer Annemieke C , Van den Ouweland Jody , Eisenhofer Graeme , Keevil Brian G , Rauh Manfred , Vogeser Michael , Pagotto Uberto

Background: LC-MS/MS is replacing immunoassays for serum steroid measurement in clinical labs worldwide. The intrinsic high specificity of LC-MS/MS is supposed to guarantee accurate quantitation, however, differences in pre-analytical and analytical procedures, and overall performance may influence the result. A few studies so far investigated the reproducibility among different LC-MS/MS assays for sex steroids. No data are available for adrenal steroids.<p class="abstext"...

GC-MS-based quantitative steroid signatures reveal different androgenic pathways between fetal and adult mice

Han Soyun , Kim Jae-Hong , Choi Man-Ho

Although the steroidogenesis in testis occur in a paracrine rather than in an endocrine, the comparative metabolic pathways between fetal and adult mice are not fully understood. Here, gas chromatography-mass spectrometry-based assay for profiling of 23 androgens, 7 estrogens, 13 corticoids, and 14 progestagens has been developed and applied to quantify their testis levels from both fetal and adult mice. Testis samples were purified with Oasis HLB solid-phase extraction and se...
